a referral describing your dental issue along with the remarks of the treating physician;
a plan of treatment, providing you have already undergone such;
We kindly inform that all the Patients referred to a periodontal examination from a different physician must have a doctor’s referral.
The results of consultation are handled to the Patients in writing. They can be also send to the treating physician by e-mail (in that case we would need the doctor’s email on beforehand).
