We hereby wish to inform you that the Personal Data Administrator is:

In order to provide medical services on the basis of the resulting obligation
from the Regulation of the Minister of Health of November 9, 2015 on the types and scope
and specimens of medical documentation and the method of its processing (Journal of Laws of 2015, item 2069)
and the Act of November 6, 2008 on patient's rights and the Ombudsman for Patients' Rights
(consolidated text: Journal of Laws of 2016, item 186, as amended), will be processed
Your personal data from the following categories:
• basic identification data: name and surname, date of birth, gender,
PESEL, home address, telephone number, e-mail address,
• health data (diagnosis and treatment, including photographic and X-ray images),
• e.g. video recordings - from video monitoring (only waiting room),
• for example, we may share your data with the following categories of entities:
- accountant - in the scope of issued bills for treatment,
- prosthetic laboratory - in the scope of transferring the order for execution
prosthetic work in the territory of the Republic of Poland and outside of Poland / EU / European
Economic Area,
- law firms in the field of pursuing claims and legal services,
- IT services in the field of failure removal and control services.

We store your data obtained in order to provide medical services
for a period of 20 years, and in the case of minors, for a period of 22 years. Monitoring recordings
are kept for a period of 3 months.

You are entitled to:
• obtain confirmation as to whether we process your personal data, and if so
to access this data, after verification of identity,
• access to your personal data and receipt of a copy of it, after verification of identity,
• to the first copy of the processed data free of charge, the issue of subsequent copies is payable,
• to be informed about the extension of more than one month of the deadline for considering a person's request,
• information that the facility is monitored and that video and sound monitoring is taking place,
as well as at the entrance to the premises of the unit there is an information plate about video monitoring,
• planned change of the purpose of processing, for a purpose other than providing medical services,
• rectify (correct) your personal data,
• being forgotten does not apply to personal data processed
based on Article. 9 sec. 2 lit. h GDPR, in particular data processing as part of documentation
throughout the required archiving period in accordance with Art. 17 sec. 3 lit. b GDPR,
• if the data are processed on the basis of the patient's consent (given for a different purpose
than providing medical services) you can exercise the right to be forgotten
to the extent specified in the consent,
• the restriction of data processing does not apply to you
personal data processed for the purpose of providing medical services, in particular
as part of medical documentation, we can process data in the current scope,
because it could significantly hinder the realization of this goal,
• withdrawal of consent to the processing of personal data; you can at any time
withdraw consent to the processing of personal data for marketing purposes,
educational, scientific, advertising - if you have given us such consent in writing;
withdrawal of consent will not affect the lawfulness of the processing we have carried out
on the basis of your consent prior to the withdrawal,
• making medical documentation available on the terms specified in Art. 26 and 27 of the Act
of the Act of November 6, 2008 on the rights of patients and the Ombudsman for Patients' Rights
(consolidated text: Journal of Laws of 2016, item 186, as amended),
• lodging a complaint to the supervisory authority; if you believe that we are processing you
personal data unlawfully, may submit a complaint to the President of the Office for Protection
Personal Data. Identity is verified by checking the information provided by

identifying the Patient. Document confirming identity is, in particular, an ID card,
student ID, driving license, passport. We can record information about verification via
entering in the documentation: the date of verification of the identity and document data,
on the basis of which the verification was made, with the simultaneous indication of the number or
ID of this document (e.g. ID number and series). In the event that
a statutory representative acts on behalf of a minor patient, the patient's identity may
also be confirmed by a legal representative
